Milan Malpensa is northern Italy's main intercontinental gateway — it is where most long-haul flights heading for Lombardy and Lake Garda land. The lake, though, is east of Milan while Malpensa is west: the drive takes about 2 hours, against roughly an hour and a half from Linate.
That extra distance is exactly why the choice of transport matters more here than on shorter routes. There are three options: the train (with two changes before the final leg), a metered taxi, or a private driver booked in advance at a fixed fare.
